Fix various problems with reflow
...
- No longer reflow lines where the full unwrapped line contains the cursor
- Add guards to prevent y and ybase becoming invalid values
Part of Microsoft/vscode#67364

Make reflow small crazy fast
...
This messy but this drops 100000 scrollback reflow from 87 cols to 40 cols
go from ~18 seconds to < 1 second, 10000 takes around 70ms
