Marek Libra b67b65cd1e Replace inline-styles by external CSS 
Selected inline-styles were externalized to xterm.css.

The `Terminal._charSizeStyleElement` is removed since it
was not used anymore.

Daniel Imms 4443ed69c7 Add noUnusedLocals tsconfig flag, fix issues 
Two parts were commented out:
- _writeStopped: This is because _xoffSentToCatchUp references this variable
  and the feature is sort of half implemented/disabled.
- _clearChar: This is because some comments still reference this, it's part
  of the work needed to do better line redrawing.

It looks like this could have caused the inconsistent behavior with
links not activating and then working again after scrolling

Daniel Imms 29ac6a153e Fix issue with strings in wrapped spans being duplicated 
This was happening because the linkifier assumed that top-level spans
in the terminal would only contain single text nodes
