Daniel Imms d0ef868215 Refactor char atlas to enable generation with worker 
This doesn't actually add workers but it separates the code nicely
such that the minimal amount of relevant code can be pulled in by
worker code.

* Make sure character dimensions are available right after calling term.open()
* Do not assume var assignments are global anymore, webpack will isolate them
* Use DocumentFragment to build Viewport and helpers before attaching them to the DOM
* Shield term.focus call from crashing when used before term.open

Repro:
- Run `ipython` (version 6.2.1)
- Copy multiple lines of text and paste into ipython prompt, e.g.
```py
print(1)
print(2)
print(3)
```
- Before: first line would paste and run, and subsequent lines are lost
- After: all lines paste into single prompt

Daniel Imms 1b2bac7aef Add letterSpacing option 
This provides an option for fonts to be more spaced out. This is particularly useful
for fonts that can seem crammed due to not allowing floating point numbers when
drawing, such as Monoid or Operator Mono.

Daniel Imms a8174398f5 Calculate mouse coordinates using correct char dimensions 
Previously the Mouse class was using raw CSS pixels, we need CSS pixels based
on the amount of space the cells take up on the canvas
