Merge upstream/main; agent-coherence -> Cohexa-ai + Glama badge

Resolve the File Systems same-slot conflict (keep upstream's GuruPDF entry).
Rename the agent-coherence entry hipvlady -> Cohexa-ai (repo moved to the org),
move it to its correct alphabetical slot, and add the Glama score badge.
